The CareLink Network has been operating for over 15 years and is comprised of nurses throughout the nation. Our nurses specialize in working with the adult and geriatric population in home care, care management, rehab and/or community resource coordination. They perform a wide range of in-person assessments with individuals in their home environment. The assessments include a review of functional, cognitive and environmental needs including a medical history, physical examination, systems review, review and assessment of potential gaps in service, and review of any available medical records.
Registered Nurses
We are seeking Registered Nurses to conduct in-person geriatric assessments on an as needed basis. Requirements include a Registered Nurse Degree, a current license in good standing as a RN in your resident state, and a minimum 3 years' experience working with a geriatric population in a home care or non-acute setting.
